Mail-in
Send your completed registration form and payment (check, money order, VISA or MASTERCARD number only, no cash will be accepted) to: Summit Park District 5700 S. Archer Road Summit, IL 60501.
Drop -off
Bring your completed registration form and payment (cash, check, money order VISA or MASTERCARD) directly to the Park District office at 5700 S. Archer Road. Office hours are Monday-Friday 8:30 a.m.-5:00 p.m. For your convenient after hours registration a mail slot is located on the east side of the office.
Fax
Fax your completed registration form and credit card payment information (VISA or MASTERCARD only) to 708-496-7275. The registration form must be properly signed to be processed. If it is not signed it will be returned unprocessed for signature.
NO TELEPHONE REGISTRATION WILL BE ACCEPTED.

GENERAL INFORMATION
1. Carefully complete the registration form. Enter the code number on the left-hand column on the form. Use the form for all free and paid programs. Please print and verify the information is correct.
2. THE WAIVER FORM MUST BE SIGNED FOR THE REGISTRATION TO BE PROCESSED.
3. Add up the fees and write the total in the appropriate space. Check or money orders should be made payable to the Summit Park District. Please write your telephone number on the check.
4. YOU ARE REGISTERED UNLESS WE INFORM YOU OTHERWISE.
5. Program registration is monitored throughout the season and programs will be cancelled if there is insufficient enrollment at least two days prior to the program beginning. The Summit Park District retains the right to open and close program registrations at its discretion.
6. If you register for a class but are unable to attend, please notify the Summit Park District at 708-496-1012 as soon as possible to cancel your registration. Other patrons may be waiting to join the class.
7. Please do not bring children to adult programs.
8. If a program reaches its maximum enrollment, the class will be “closed”. A waiting list is then started for those interested in registering. When and if an opening occurs in the program, upon the instructor’s approval, the first person on the list will be contacted. We will continue down the list if more vacancies become available. Do not include payment with your registration form if you are put on a waiting list. The registration fee will be collected after you are contacted.
9. The Summit Park District retains the right to assign participants on the waiting list in the order the names are listed. Participants cannot give their “spot” to anyone else.
10. The Summit Park District is not responsible for any omissions or typographical errors.
11. The Summit Park District does not carry accident or hospitalization insurance for any program participant. All insurance coverage is the participant’s responsibility.
CANCELLATION AND REFUND POLICY
Refunds of 100% are made, less a $3.00 service charge, if you notify the Park District of your cancellation at least two working days prior to the start of the program. A refund of 50% is given if you notify the Park District of your cancellation prior to the second course meeting. No refunds will be given after the second course meeting. No refunds are given if you notify the Park District of your cancellation the day of a one-day program, trip or event. If you cancel your participation in a program due to medical reasons, a signed physicians authorization is required before a refund can be issued. If the Park District must cancel a class, a full refund will be given.
RESIDENT REGISTRATION BEGINS December 27
Beginning Wednesday, December 27 all resident registration forms received via drop-off, in-person, mail, or fax will be drawn and processed at random. Thereafter each day’s mail will be drawn and processed at random.
NON-RESIDENT REGISTRATION BEGINS JANUARY 3rd Beginning Wednesday, January 3rd all non-resident registration forms received via drop-off, in-person, mail, or fax will be drawn and processed at random. Thereafter each day’s mail will be drawn and processed at random.
Non-residents pay one-and-a half times the resident program rate. Non-residents may register for a program that is free to residents but they will be charged a fee based on actual program costs.
